Before leaving the hospital
- Discharge summary with diagnosis, procedures and medicine list.
- Which old medicines to stop.
- Follow-up date and the doctor’s contact.
- Warning signs that need a call back.
First 7 days at home
- Someone with them for the first 48 hours.
- Medicines started exactly as written — photograph the new list.
- Daily check-in and readings if advised (BP, sugar, temperature).
- Food, fluids, toilet and mobility help arranged.
Call their doctor the same day if
- fever, new breathlessness or chest pain,
- wound redness, discharge or bleeding,
- confusion, fainting or a fall,
- unable to eat, drink or pass urine.
Follow-ups
Book the follow-up visit and any tests before you fly back. Keep the discharge summary with the emergency card.
